Chapter 1: The Historical Context of Needle Use

Let's face it: the hypodermic needle isn't exactly a favorite among patients. While I don't suffer from a fear of needles, who genuinely enjoys the experience of receiving an injection, whether in the arm or elsewhere? One has to ponder if there's a certain grim satisfaction that medical professionals derive from this process, or perhaps it’s an outdated form of medical tradition. After all, for nearly 178 years, the needle and syringe have remained largely unchanged. It's astonishing, especially considering our advancements, like space travel!

The first hollow needle injection was recorded in 1844, utilizing a rudimentary one-piece metal design with a rubber plunger. The size of that needle is beyond comprehension! Thankfully, we've progressed to a modern, disposable two-part system, consisting of a plastic syringe and a retractable needle. Yet, the core issue remains: there is still a needle involved.

With the COVID-19 pandemic, the frequency of vaccinations—every 4 to 6 months—has become a new norm. While I’m willing to do my part for public health, I often wonder if we might soon see a needle-free injection technology (NFIT) that could transform how we receive vaccines.

Section 1.1: Inspirations from Science Fiction

Science fiction offers a wealth of inspiration for innovative solutions. For example, the Hypospray from Star Trek allows for quick, painless drug administration without any needles. Research and development of jet-like injectors similar to the Hypospray have been underway since the 1970s. These devices played a crucial role during the 1976 Swine Flu outbreak for mass vaccinations. However, concerns about disease transmission led the World Health Organization to halt their use.

Meanwhile, scientists are investigating various advanced delivery mechanisms, such as gas propellants, Lorentz forces, lasers, and shock waves. Concurrently, there are efforts to enhance formulations that can penetrate the skin under pressure. Unfortunately, none of these innovations have reached the commercial market yet.

Subsection 1.1.1: A Glimmer of Hope

Vaxxas needle-free microarray patch

There is, however, promising news on the horizon. An Australian company named Vaxxas is developing a high-density microarray patch (HD-MAP) that eliminates the need for needles altogether. This innovative patch features thousands of tiny (approximately 0.24 mm) micro-projections that can be infused with a vaccine.

To use it, one simply presses the patch against the skin for about 10 seconds to deliver the dose. Importantly, it can be self-administered at room temperature. According to Vaxxas’ website, the HD-MAP efficiently delivers the vaccine to the immune cell-rich areas just beneath the skin's surface. The micro-projections also activate natural immune responses, prompting swift transportation of vaccine components to lymph nodes, thereby generating a strong immune reaction.

This patch does not require costly refrigeration during transport or storage, and once the vaccine is administered, it can be easily recycled or disposed of.

Chapter 2: Future Prospects and Final Thoughts

Early trials suggest that the patch effectively delivers the SARS-CoV-2 spike vaccine, with final trial results expected in early 2022. Let’s hope this technology receives the necessary FDA approvals and becomes available to the public before my next booster shot. It's high time we retire the needle for good.
